Parent Notification Letter
To be sent to parents of all students scheduled to be screened.

To be sent to School Nursing Solutions 4 weeks before screening date.

To be printed, cut, and completed. Printed labels are recommended.
What to expect for round one.

Duration
• Our team will arrive at 8:45am
• Setup typically takes 15-25 minutes depending on the space provided
• After the screening, our team will take anywhere from 30 minutes to 2 hours to complete the data
• Wifi connection issues may cause delays

Volunteer Responsibilities
• Pull entire classes at a time
• Ensure all students have a Student Slip to give to the screener.
• Keep a steady flow of classes - as one class gets close to finishing up, another class can be called to the screening room
• Help funnel students into the screening room, to the screening station, and to the exit as needed
• Minimize noise inside and outside of the screening room

Screening Sequence
• 1 student is screened at a time
• The rest of the class is waiting quietly outside of the screening room
• As the student leaves the room, the next student can enter
• As one class gets close to finishing up, another class can be called to the screening room

Completion
• A Completion Slip must be signed by our team and an on-site contact
• Student Slips & Completion Slip are delivered to on-site contact and are ready to be filed
• Student Slips for those absent in Round 1 should be saved and used for Round 2
• Round 2 will be scheduled shortly after Round 1 has been completed
• Each school site is given a password protected link with all final deliverables
What to expect in round two.

Students to Screen
• A Round 2 is needed to ensure compliance. In this round, we will screen students who were absent and who failed hearing in Round 1 to rule out false fails due to congestion.
• All students with a checkmark in the 2 most left columns of the Comprehensive Sheet (found in your password protected link) must be screened
• All students who were absent during Round 1 must be screened - You may refer to the unused Student Slips from Round 1

A Google Sheet of all recorded data from the screening, organized and consolidated into one file
Student Slips
A digital copy of all the physical Student Slips that were delivered at the end of each round
Completion Slip
A digital copy of the Completion Slip (a receipt of the screening completion and Student Slip delivery) that was delivered at the end of each round
Referral Letters
A Round 1 and Round 2 folder containing student Referral Letters from their respective rounds - Ready to be emailed or printed and sent out
