From Compliance to Care: The System Nobody Actually Built

Most school health departments were never designed. They were inherited.

Somebody, years ago, set up the school nurse role a certain way. Maybe it was based on what the district could afford at the time. Maybe it was copied from a neighboring district. Maybe it just reflected whatever the person in the role before you decided to do, and nobody ever asked if it still made sense.

That structure got passed down. New nurses stepped into it. New directors inherited it. And every year, it kept running, not because anyone confirmed it was working, but because nobody stopped to check.

Here’s the problem:

The times have changed. And we need to change with the times.

Student health needs have changed. Caseloads have changed. IEP demands have changed. But in a lot of districts, the school health department is still running on a design from decades ago, built for a different set of kids, a different set of laws, and a different school system entirely.

This isn’t a story about bad intentions - most people running school health departments are doing their best with what they’ve got. The real issue is that almost nobody was ever handed a system built on purpose. They were handed what accumulated.

That accumulation is not intentional.

A department that accumulates, flattens.

Multiple nurses, all doing the same tasks, at every site, the same way, regardless of what that specific school actually needs. Nobody specializes. Nobody builds. The department responds, but it can’t improve. Because it was never built to.

If you lead a school system and are honest with yourself, you already know if your school health department was designed or inherited. You know if it was built to meet today’s needs or whether it’s remnants of the structure that happened to survive.
